What we looked for: overall healthcare quality and access, scored 0–10 for every city in the database. The score weighs the strength of local hospitals (CMS star ratings, Leapfrog safety grades, U.S. News rankings), the depth of specialty care for the things that matter most in retirement β€” cardiac, cancer, neurology β€” and how far you'd realistically travel when a routine checkup turns into something serious. Scores come from the database; the hospital names and rankings in each card are supporting detail and worth re-verifying against current sources before any healthcare decision.

Four things to know about retirement healthcare.

Scottsdale stands alone. Mayo Clinic Arizona β€” consistently ranked among the top hospitals in the country β€” sits in north Scottsdale. No other retirement city in the database offers on-site access to a hospital of that caliber. If healthcare is your single highest priority, it's the clearest answer on the list.

Florida's healthcare picture is wildly uneven. Naples, Sarasota, and Tampa score a perfect 10 β€” genuinely exceptional hospitals. But elsewhere in the state, the local hospital can carry a failing safety grade. The state you retire to matters far less than the specific hospital serving your zip code, and Florida proves it more than anywhere.

The Northeast surprise is value, not quality. Philadelphia and Pittsburgh both score a perfect 10 on healthcare, anchored by Penn Medicine and UPMC β€” and they do it at some of the lowest housing costs of any major city in the database. World-class academic medicine without the coastal price tag is a combination almost nothing else on the list matches.

The mountain trade-off is real. The cities that score highest for scenery and outdoor recreation β€” Sedona, Jackson Hole, Flagstaff β€” score lowest for healthcare. Sedona has no hospital at all; Jackson Hole's nearest serious specialty care is hours away over mountain passes that close in winter. The beauty premium comes with a genuine healthcare cost that retirees should accept consciously.
